Someone suggested that, as it seems like the scene where River gave up her regenerations to save the Doctor's life might well have been put in to get around the 12 regeneration limit, that when he reaches the end of his own lives and moves on to regenerations acquired from River, he could then switch sex and or gender. It's an interesting idea, but I'm not sure the show-runners, whoever they may be at that point, would actually go for it*, and I can also see lots of ways in which it could be done incredibly badly.
* If only because it's a bit too reminiscent of "The Curse of Fatal Death".
